大陸地區人民專案申請來臺接受醫療服務

◎適用對象：
（一）因罹患目前中國大陸醫療技術無法醫治之疾病，申請來臺就醫之大陸地區人民。
（二）申請來臺接受

nbsp

「活體肝臟移植手術」、「顱顏重建手術」、「心血管侵入性治療及外科手術」

「人工生殖技術」、「關節置換手術」 nbsp 等5項醫療服務之大陸地區人民。

nbsp

◎應備文件（1式3份）：
（一）申請書：每人填寫大陸地區人民入出臺灣地區申請書，並貼最近6個月內所拍攝、直4.5

nbsp nbsp nbsp nbsp nbsp 公分且橫3.5公分、脫帽、未戴有色眼鏡、五官清晰、不遮蓋、足資辨識人貌、人像自頭

nbsp nbsp nbsp nbsp nbsp 頂至下顎之長度不得小於3.2公分及超過3.6公分、白色背景之正面半身薄光面紙彩色照

nbsp nbsp nbsp nbsp nbsp 且不得修改或使用合成照片。
（二）大陸地區居民身分證影本、其他證照或足資證明身分文件。
（三）就診醫療院所同意函正本（表明願意提供相關醫療服務與負擔申請人因違規違常遭強制出

nbsp nbsp nbsp nbsp nbsp 境及收容時之相關費用）。
（四）醫療計畫及療程表。
（五）病歷紀錄（應包含病歷摘要、檢查或檢驗報告，若為影本則需加蓋與正本相符章。申請項

nbsp nbsp nbsp nbsp nbsp 目為活體肝臟移植手術者，需另檢附捐贈者之病歷紀錄；申請項目為人工生殖技術者，

nbsp nbsp nbsp nbsp 需另備夫妻雙方不孕症檢查之相關證明資料）。
（六）下列申請來臺大陸人士之親屬關係證明文件（須在中國大陸辦理公證，並經財團法人海峽

nbsp nbsp nbsp nbsp nbsp 交流基金會驗證，詳第十一點之（四））：

nbsp nbsp nbsp nbsp nbsp １、陪同來臺家屬
　 nbsp nbsp nbsp ２、申請活體肝臟移植手術之捐贈者及受捐贈者
　 nbsp nbsp nbsp ３、申請人工生殖技術之夫妻

（七）出具於大陸地區金融機構有等值新臺幣二十萬元以上存款之證明，或出具固定正當職業

nbsp nbsp nbsp （或學生）之證明。
（八）其他相關證明文件：

nbsp nbsp nbsp nbsp nbsp １、隨同（機）照料醫護人員：來臺必要性說明、名冊及醫療專業證明文件。
nbsp nbsp nbsp nbsp nbsp ２、因罹患目前中國大陸醫療技術無法醫治之疾病，申請來臺就醫者需另檢附：
nbsp nbsp nbsp nbsp nbsp （１）來臺醫療之必要性說明
nbsp nbsp nbsp nbsp nbsp （２）非傳染病說明
（九）其他經相關機關要求之證明文件。
（十）證照費申請人每人新臺幣400元整。
◎申請方式：由臺灣地區之就診醫療院所代向所在地入出國及移民署各縣市服務站送件申請

nbsp （不受理郵寄申請）。
◎核發證件種類及效期：核發單次入出境許可證，入出境許可證之有效期間自核發之日起算為

nbsp nbsp 6個月。
◎核給方式：通知代申請醫療院所領取入出境許可證正本，再轉交申請人持憑經機場查驗入出

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 境。
◎停留期間：大陸地區人民申請來臺接受醫療服務，其停留期間依醫療需要覈實發給，但不得逾

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 3個月，必要時得延長之，延長後每次停留期間不得逾1個月。

◎延期方式：大陸地區人民入境後，其申請原因繼續存在，而有延期必要者，應於停留期間屆滿

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 十日前，備齊下列文件向入出國及移民署各縣市服務站申請：（不受理郵寄申請）
　　 nbsp nbsp （一）延期申請書。
　　 nbsp nbsp （二）入出境許可證正本。
　　 nbsp nbsp （三）就診醫療院所出具確有延期必要之證明文件。
　　 nbsp nbsp （四）申請人每人新臺幣200元整。

◎申請回診：申請人若有回診必要時，應檢具相關病歷資料並說明回診必要性，重提入境申請。

nbsp nbsp nbsp nbsp nbsp nbsp nbsp 對於申請回診者，主管機關得視實際需要發給逐次加簽入出境許可證。

◎注意事項：

nbsp nbsp nbsp nbsp nbsp nbsp （一）代申請醫療院所若發現申請人及其陪同家屬在臺期間發生違規違常情事，應立即通

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 報衛生署。
nbsp nbsp nbsp nbsp nbsp nbsp （二）陪同來臺家屬限於申請人之二親等內血親、配偶、配偶之父母、或子女之配偶，且

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 以1人陪同為原則（惟得依醫療需要申請2人同時或先後來臺陪同）：

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp １、如屬來臺進行活體肝臟移植手術，其肝臟捐贈者與受贈者皆為申請對象，故各可

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 申請陪照護家屬1人為原則（惟得依醫療需要申請2人同時或先後來臺陪同）。
nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp ２、陪同來臺家屬應檢附第三點第（一）、（二）、（六）及（十）項之應備文件，

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 由醫療院所併案申請之。
nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp ３、有特殊原因需申請其他三親等內親屬陪同來臺，應詳述原因並檢附必要證明文件

nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp nbsp 申請專案許可。
